Employees who regularly work more than 4 hours per day are eligible for health insurance through Empire or HIP. The Open Transfer Period for changing between the two plans is November 1st through December 22nd. Please contact Personnel for more information.
Employees covered by UTIT and CSEA Units are eligible for Health Insurance Opt-Out. A letter of request must be received each year by the Asst. Supt. for Business no later than March 1st for CSEA Custodial Unit and May 15th for UTIT and CSEA Clerical Unit. Beginning this year, the Opt-Out request must be accompanied by proof of insurance coverage though another plan. Effective January 1, 2013, State requirements will not allow us to make Opt-Out payments to those employees who are covered through Empire.
All employees are eligible to enroll in AFLAC’s insurance coverage, at their own expense, through payroll deductions. Enrollment and changes must be made prior to January 1st through an AFLAC representative. AFLAC will arrange to visit each building to meet with interested employees.
Employees covered by the Admin., UTIT, and CSEA Clerical Units are eligible to enroll in Medical and Dependent Care Flex Savings Plan. Contributions are tax free. Enrollment and changes must be made prior to January 1st. Enrollment forms must be completed each year and will be available in all buildings during late November. Forms should be returned to Payroll on or about December 15th each year for processing.
UTIT Salary Advancement:
Employees under the UTIT contract are eligible for Salary Advancement through completion of approved college and in-service credits. Proof of course completion and letter of request must be received by Personnel by October 1st and March 1st for salary advancement retro to September 1st and February 1st respectively. Please refer to the Salary Advancement section of your contract for further information.

Dental Benefit Details
If you are currently enrolled for Dental Coverage and you wish to continue Dental Coverage for
the 2017-2018 school year:
You must write a check payable to UTIT Welfare Fund in the amount of:
$150 for Individual coverage
$240 for family coverage.
Check is due to your building rep by June 2,2017.
You do not need to fill out an enrollment form if you presently have coverage.
If you have a change of life event, an enrollment form is necessary.
**The new maximum allowed per family, effective 7/1/11 is $6,000.00 per year.
This is not part of the $3000 orthodontist lifetime coverage per person.
Despite the health care changes brought under Obama's administration, the UTIT Dental
coverage for dependents will remain the same as it has in the past. Dependents, under the age
of 19, who still reside with a participating member, are entitled tocoverage.
Please pay for family coverage. For dependents enrolled in college, their dental coverage
will continue until they reach their 26th birthday.

Dental Coverage Eligibility and Late Enrollment Penalties
Dental Coverage Eligibility:
Teachers become eligible for dental coverage on the first day of September of their second year
employed in Island Trees SD. Employment for a full year is not necessary. Only one day of work
is required for eligibility for dental coverage in the second year of employment.
Coverage for you and/or your dependents cannot begin before you enroll.
Appropriate forms and required payments must be submitted by May 25th of the employees’ first year working in ITSD.
Members that do not sign up for dental coverage promptly, can sign up at any time after becoming eligible,
but must understand that late enrollment has a consequence of limited coverage for the first 24 months of coverage.
Below are the details taken from the dental handbook that outlines the penalties for not enrolling promptly.
The intent of the penalty is to prevent any member from not paying for dental coverage only to suddenly enroll when a
need for coverage arises.
Late Enrollment Limitations and Penalties:
Anyone who enrolls after 31 days after becoming eligible for dental coverage, will receive NO COVERAGE until YOU or such DEPENDENTS has been covered for:
- Six months under Diagnostic and Preventive Services, other than routine oral examinations and x-rays AND
- Twenty four months with respect to Periodontal, Major and Orthodontic Services.
